Ten weeks after Congolese authorities declared an Ebola emergency in the northeastern province of Ituri, the epidemic has crossed a mortality threshold that the West African outbreak of 2013 to 2016 took roughly eight months to reach. Al Jazeera reported on 22 July that confirmed deaths in the Democratic Republic of the Congo had reached 1,031, against 2,473 confirmed cases, with 737 patients then in isolation or hospital care.
The most recent formal situation assessment published by the World Health Organization, dated 17 July and covering data through 15 July, counted 2,124 confirmed cases and 828 deaths in the DRC, a case fatality ratio of 39 percent. Adding 20 confirmed cases and two deaths in Uganda, one recovered case in France and one medically evacuated patient treated in Germany, the WHO put the cumulative regional total at 2,145 confirmed cases and 830 deaths, with 410 recoveries.
Aggregated tallies of Congolese health ministry reporting put the DRC figures at 2,536 confirmed cases and 1,033 deaths as of 21 July, a fatality ratio near 41 percent. Whichever series is used, the trajectory is the same. At its media briefing on 16 July, the WHO said the epidemic had become the third largest Ebola outbreak recorded and that over the preceding month it had expanded faster than any previous outbreak of the disease.
Strain Outside the Existing Arsenal
The defining constraint of this emergency is virological rather than logistical. The pathogen is Bundibugyo ebolavirus, the rarest of the four species known to cause disease in humans, and not the Zaire species against which the world's countermeasures were built. Ervebo, the single-dose vaccine licensed after the 2018 to 2020 epidemic in North Kivu, and the two-dose Zabdeno and Mvabea regimen both target Zaire ebolavirus. According to the WHO, neither is considered protective here, and the agency has advised against deploying them in this Bundibugyo Ebola outbreak.
The same gap applies to therapeutics. The monoclonal antibody products approved in the United States for Ebola treatment were authorised against Zaire virus. In their absence, clinicians in Ituri have relied on isolation, contact tracing and supportive care. A randomised clinical trial opened in the DRC on 2 July evaluating the broadly reactive monoclonal antibody MBP134 alongside the antiviral remdesivir, with more than 1,200 treatment courses reported to be available.
Vaccine work has been accelerated on a compressed timetable. Three candidate programmes received expedited funding, with roughly 3.2 million dollars directed to IAVI, 50 million dollars to Moderna and 8.6 million dollars to a University of Oxford effort. None of that will produce licensed doses inside the current transmission window. The practical consequence is that a response operating at emergency scale is doing so with tools that predate the specific threat.
Ituri Absorbs the Epidemic
Geographic concentration has been extreme. The WHO recorded 46 health zones across five provinces with confirmed cases, but Ituri alone accounted for 1,904 of 2,124 Congolese cases, or 89.6 percent, and 692 of 828 deaths, or 83.6 percent. Transmission remained active in 38 health zones that had reported cases within the preceding 21 days. The heaviest burdens sat in Bunia with 570 cases, Rwampara with 418 and Mongbwalu with 347.
Contact tracing figures illustrate where the response is holding and where it is not. The WHO listed 12,693 contacts under follow-up. Completion rates were 91.7 percent in North Kivu and 78.1 percent in Ituri, but only 50 percent in Tshopo. A province where half of listed contacts go unmonitored is a plausible seed for the next cluster, and Tshopo sits on the western flank of the epicentre, along river and road corridors that connect to more populous areas.
Burial practice has emerged as a second amplification channel. The International Organization for Migration has warned that transporting the bodies of victims between districts for family burials risks carrying the virus into communities that had none. Surveillance teams have intercepted 105 transported bodies and referred them for testing and safe burial, a figure that indicates both the scale of the practice and the limits of interdiction.

Health Workers as the Leading Indicator
Infection among clinical staff is the standard proxy for whether infection prevention is functioning. The WHO counted 119 confirmed cases among health workers, with 61 recoveries and 36 deaths, a fatality ratio of 30.3 percent. Losing that many trained personnel in ten weeks compounds every other constraint in a province where the pre-outbreak clinical workforce was already thin.
Capacity itself has expanded quickly. Briefing reporters in Geneva on 24 June, WHO Director-General Tedros Adhanom Ghebreyesus said treatment beds had gone from fewer than ten to more than 500 across 19 health centres, while laboratory throughput rose from 30 tests a day at the central laboratory in Kinshasa to more than 2,000 a day across nine laboratories in three provinces. At that point the confirmed toll stood at 1,094 cases and 277 deaths. Cases have roughly tripled since.
Medecins Sans Frontieres, which has more than 1,400 staff deployed in the DRC, called on 15 July for an urgent scale-up, arguing that control efforts remained insufficient against the pace of spread. Thierno Balde, the WHO incident manager, has described the outbreak as still ahead of the response, with teams in a catching-up phase.
Financing Arrives Late and Unevenly
Money has moved, but slowly and from a narrow base. Reported commitments include roughly 20 million pounds from the United Kingdom, 112 million dollars from the United States and 15 million euros from the European Union. Analysts tracking the response have noted the reduced American footprint relative to previous Ebola emergencies, following the dismantling of USAID structures and Washington's withdrawal from the WHO.
The multilateral backstop is itself under strain. The WHO is carrying a funding gap of well over a billion dollars against its planned 4.2 billion dollar budget for 2026 and 2027, a shortfall Health Policy Watch has reported runs to roughly 2.5 billion dollars once the full budget period is counted, on top of an envelope that had already been cut. Jean Kaseya, director-general of Africa CDC, framed the shortfall bluntly in comments reported by Al Jazeera, saying the outbreak "is escalating at an alarming pace" and linking deaths directly to the absence of vaccines, medicines and funding.
Border measures have tightened in parallel. The United States Centers for Disease Control and Prevention renewed entry restrictions and airport screening for travellers arriving from the DRC, Uganda and South Sudan through mid-August, having first imposed them earlier in the summer. The WHO grades the risk as very high inside the DRC, high in Uganda and neighbouring states, and low globally.
Racing a Countdown Nobody Controls
One indicator points the other way. Uganda identified its last confirmed case on 21 June and discharged its final patient on 16 July after two negative tests, starting a 42-day countdown that, if uninterrupted, would end its chapter of the epidemic in late August. No community transmission was documented there, which suggests cross-border importation was caught rather than seeded.
The Congolese picture offers no equivalent clock. With active transmission in dozens of health zones, contact follow-up incomplete in at least one province, no licensed vaccine and no licensed therapeutic, the epidemic curve is being bent by case management and behaviour change alone. That combination has worked before, but never against a strain this poorly matched to the available stockpile and never with this speed. The next four weeks of data from Ituri, and in particular the health worker infection count, will indicate whether the Bundibugyo Ebola outbreak is being contained or merely counted.
